Resources for EMERGENCY AND End-of-Life Planning ​

Planning for the future is hard, but absolutely necessary. The resources below can help you create documents and take the actions that you will need to have a say in what happens in emergency situations or at the end of your life. 
Download and complete these resources and then bring your completed documents to the Annual Chicago Theater Bike Ride & BBQ to have them notarized. You'll receive a Love Hard document bag to keep your documents in. Put the bag in a safe place and be sure to tell your loved ones where it is!

​Powers of Attorney
Powers of Attorney are needed to allow someone else to handle your affairs while you are still alive. These documents become invalid at death. Illinois government agencies created template forms for Health Care and for Property. They can be found at these links:
Wills
There are no standard templates for creating a will in Illinois. If a will meets all the requirements, then it is valid. There are online programs available to help create a will. It is recommended that you have a lawyer review your will to verify its validity.
